To register as a BoddFit member, please read the Terms & Conditions and complete the following steps:

Membership Form: Complete the webform below or access the PDF version HERE. If you choose to complete the PDF form, email the completed document to shire@boddington.wa.gov.au
Direct Debit Form: Upon signing up through the webform below, please ensure to attach the completed Direct Debit form, accessible HERE. (Relevant only for sign-ups via the webform)
Pre-Exercise Screening Form: Access the form HERE.
Gym Induction: Contact the Shire Administration by phone (08) 9883 4999 during office hours to obtain contact details for BoddFit-authorised Personal Trainers, and then schedule the Gym Induction with the Personal Trainer.
Swipe Card: After completing your induction your Personal Trainer will provide you a swipe card which will allow you to officially start your membership. The membership start date will recognised from the date of the induction completion date.

The Shire of Boddington collects personal information to process requests, applications and related enquiries. Information may be shared with relevant parties where required or as authorised by law. If the information is not provided, the request or application may not be processed. Further information is available in the Shire's Privacy Statement.
